Financial Policy
Payments may be made by cash, check, money order, Visa, MasterCard or Discover. All patients 18 years of age and older are considered guarantors of their account unless legal documentation indicates otherwise. In cases of separation or divorce, we ask that both parents discuss payment arrangements prior to the appointment. The parent bringing the child for treatment will be named guarantor of the account since we cannot allow an individual to assign payment responsibility to someone who is not present.
Insurance contracts are agreements between patients and their insurance companies. It is the patient’s responsibility to be aware of contract benefits and limitations. We are happy to submit claims to your insurance company, but understand that unpaid account balances after 60 days are considered due at that time. (Implant fees adhere to a different payment schedule and are discussed prior to implant surgery).
If we are providers for your insurance company, 20% payment is required on the date of service. When predetermination of benefits has been done, the indicated patient responsibility will be due on the day of service. If we are not providers with your company, we require 1/3 payment of the total fees at the time of surgery. If there is no insurance coverage, payment in full is due on the day of service.
Patients wishing to finance treatment fees over a longer period of time may qualify for a commercial option our office offers. Please request details from our accounts coordinator prior to your surgery date.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
